How to schedule
Click on the “Request an Appointment” and fill out the form. A member of our staff will call or email you ASAP to get you scheduled. *If there is a specific doctor you would like to see, please be sure to let the front desk know when you are scheduling your appointment. If there is no preference, we will match you with one of our wonderful chiropractors.
Forms Required
Once an appointment is confirmed, the front desk staff will email you a link to complete the new patient paperwork. If you are unable to complete the forms online, we can print out a copy for you to fill out in person. Please arrive 15 minutes early in this case so you have time to fill it out!
What to bring?
Please bring your driver’s license and insurance card. We will take a copy of these when you arrive.
What should I wear?
Comfortable clothing that you can move around in is always recommended, however not a necessity.
What to expect?
During the first chiropractic visit, the doctor will take a detailed health history and discuss any issues that might be going on that you would like to address. They will then perform an examination to better assess and understand your current condition. Once that is complete, the doctor will talk with you about their findings. Often times, the doctor is able to start some level of treatment during that first visit. Other times, treatment will begin with a patient’s second appointment depending upon patient presentation and pending any further testing or imaging needed prior to treatment.
How long is the first appointment?
Initial consultations are scheduled for an hour to give the doctor and patient the time to fully discuss and examine the patient’s condition and treatment goals.
